Parent / Guardian Agreement

I am the parent/legal guardian of this student and certify that all information provided in this registration is true, accurate, and complete to the best of my knowledge. I give permission for my child to enroll in and participate in all educational programs, classes, school-sponsored activities, cultural events, religious activities, and other school functions organized by Mata Sahib Kaur Ji School. I authorize Mata Sahib Kaur Ji School to photograph, video record, or otherwise capture my child's participation in school activities and to use such photographs or recordings in school publications, newsletters, brochures, presentations, the school website, social media platforms, and other promotional or educational materials without compensation. In the event of a medical emergency, if I cannot be reached after reasonable efforts, I authorize Mata Sahib Kaur Ji School and its representatives to obtain emergency medical care or treatment for my child as deemed necessary. I understand that I am responsible for any medical expenses that may result from such treatment. I understand that it is my responsibility to promptly notify the school of any changes to my contact information, emergency contacts, custody arrangements, authorized pickup persons, allergies, medical conditions, medications, or any other information that may affect my child's health, safety, or education. I acknowledge that final decisions regarding class placement and grade level are made by the Principal based on the student's age, academic assessment, previous records, and the school's educational policies.
